Detailed Information for Clients

Documented:

• Household income of less than $35,000 if married.
• Household income of less than $30,000 if unmarried.
• Those with household income of around $24,000 or lower are eligible for Medi-Cal. (The cutoff varies depending on family size)

Undocumented:

• Must be less than 26 years old or more than 65 years old.
• Household income around $24,000 or less
• Would only be eligible for Medi-Cal

Proof of identity, citizenship and age OR proof of immigration status

• Current driver’s license or photo ID / a current passport
• Green card, employment authorization, etc.

Proof of current residence/home address

• For homeowners, a mortgage bill or property tax statement qualifies as proof of your address
• For tenants, a rent receipt, apartment lease contract or signed letter from your landlord

Proof of current or future income

• 1040 federal or state tax return

Social Security Number

• Social security card
